Laparoscopic Ureteroureterostomy in a Porcine Model: A Simple New Technique Using Nonperforating Vascular Clips

چکیده
Introduction: Laparoscopic suturing is a tedious procedure which has a long learning curve. Hence, we sought to develop an alternative simple and reliable technique for laparoscopic tissue approximation to enhance laparoscopic reconstructive procedures. In this video, we present our method for using newly developed, nonperforating titanium clips (U.S. Surgical Inc., Norwalk, CT) to perform a Ureteroureterostomy. Method: Ten piglets underwent a unilateral laparoscopic Ureteroureterostomy. Preoperative endoscopic ureteral stenting was first performed. After achieving a pneumoperitoneum, the retroperitoneal space was accessed, and the proximal ureter was dissected, opposite the lower pole of the kidney. Transection of the ureter was performed, and a Ureteroureterostomy was accomplished with a laparoscopic endovascular stapler (VCS). Tissue approximating forceps (AF) were used to bring the two edges of the ureter together. For application of staples to the posterior surface of the ureter, the tissue approximator forceps were used to rotate the ureter, after which VCS clips could be applied to the posterior surface. A total of 11-14 staples were applied. Results: The procedure was successful in all animals without any intraoperative complications. The operative time was significantly shorter with the use of the newly developed laparoscopic tissue approximator. Conclusions: Laparoscopic Ureteroureterostomy using VCS clips appears to be a promising method for reconstructive ureteral surgery. Clipless management of the renal vein during hand-assist laparoscopic donor nephrectomy
BACKGROUND Laparoscopic live donor nephrectomy has become the preferred method of donor nephrectomy at many transplant centers. The laparoscopic stapling device is commonly used for division of the renal vessels. Malfunction of the stapling device can occur, and is often due to interference from previously placed clips. We report our experience with a clipless technique in which no vascular cli...
متن کاملA Feasible Technique for Transient Vascular Occlusion by Using a Vessel Loop and Hem-o-Lok Clips in Laparoscopic Partial Nephrectomy
PURPOSE We introduce our transient vascular occlusion technique that uses a vessel loop and Hem-o-Lok clips in laparoscopic partial nephrectomy. MATERIALS AND METHODS From March 2009 to March 2011, 15 consecutive patients underwent laparoscopic partial nephrectomy. All operations were performed by a single surgeon using the transperitoneal approach. The transient vascular occlusion technique ...
متن کاملLaparoscopic Vascular Control Techniques in Donor Nephrectomy: Effects on Vessel Length
BACKGROUND Various techniques for vascular control have been used during urologic laparoscopic procedures. The importance of optimizing the vessel length and securing reliable vascular control are critical for procedures like laparoscopic donor nephrectomy. We aimed to determine the length of vessel lost by using 4 common techniques of vascular control in a fresh human cadaveric vascular model....

MATERIALS AND METHODS We performed a comparative experimental study between a new radiofrequency (RF)-assisted device consisting of a handheld instrument that simultaneously conducts coagulation and cutting tasks without hilar clamping vs a standard techn...
متن کاملA Laparoscopic Management Combined with a Flexible Ureteroscope for Ureteral Polyps of More Than 3 cm Length
Ureteral polyps are benign tumors of the ureter, which are relatively rare. The etiology has proposed various hypotheses, involving chronic inflammation and congenital disease. Most of them are commonly diagnosed in the upper ureter including the ureteropelvic junction. Some studies have reported polypectomy using a holmium laser, but several studies presented laparoscopic ureteroureterostomy f...
